The Bend Chamber Association Health Plan gives Chamber members access to competitive medical and dental coverage designed for Oregon employers and their employees. With Moda Health as the primary medical carrier and Kaiser Permanente available as an additional option, the program is expanding choice while continuing to provide robust benefits and competitive rates.

Look no further than the Bend Chamber Association Health Plan! With over 800 groups enrolled, covering over 14,000 lives, the plan options are growing and becoming more affordable than ever. In fact, our plans have richer benefits and are often less expensive than most small group plans in the marketplace.
Rates are based on your group’s demographics, the plan design selected, and percentage of employer contribution. Some of the underwriting guidelines you’ll need to consider:
- Groups written shall be members in good standing with the Bend Chamber (or your local chamber)
- Plans are available to groups of two or more with a minimum of 2 enrolled (family-only plans not available)
- There must be at least 1 common law/payrolled employee enrolling
- Employer must contribute a minimum of 50% of employee’s premium
- Employer may set eligibility between 17.5 and 40 hours per week
Beginning October 1, 2026, Moda Health will become the primary medical carrier for new enrollments in the Bend Chamber Association Health Plan. Kaiser Permanente will also be available to eligible employers of all group sizes within its service area.
These expanded options follow a comprehensive and competitive review led by the Bend Chamber Board of Directors and Health Insurance Task Force, working closely with our benefits partner, Alliant/Johnson Benefit Planning. The goal has been to maintain a strong, sustainable and competitive health insurance program while offering Chamber members greater choice.
Moda Health will offer medical coverage through the Bend Chamber Association Health Plan beginning October 1, 2026, and will serve as the primary carrier for new enrollments. Moda will also continue to offer dental coverage through Delta Dental and Willamette Dental.
Kaiser Permanente is available as an additional medical option for eligible employers of all group sizes. Groups with 51 or more employees may be able to offer Moda and Kaiser together in a dual-carrier arrangement. Groups with fewer than 51 employees may select either Moda or Kaiser as their sole medical carrier. Kaiser availability is subject to its service-area, participation and eligibility requirements, including applicable employee residency requirements.
The timing of each group's move from Providence Health Plan to Moda Health will depend on its renewal date:
Important: Groups transitioning to Moda before January 1, 2027, will receive credit for eligible deductible and out-of-pocket maximum amounts accumulated in 2026. Rates will be guaranteed for 12 months.
The Bend Chamber Association Health Plan is available to eligible Oregon employers of varying sizes. Plan availability and requirements may differ by carrier and location.
